Synopsis: The Doctor. Kenzou Tenma, an elite neurosurgeon recently involved with her hospital director's daughter, is on her way to climbing the hospital hierarchy. This is until one night, a seemingly small event changes the life of Dr. Tenma forever. While preparing to surgery on someone, he receives a call from the hospital director, telling him to change patients and instead perform brain surgery that saves lives in a famous artist. His colleagues, bride, and the director of the hospital applaud his achievement; But because of the change, a bad immigrant worker is dead, making Dr. Tenma has a crisis of conscience.

So when a similar situation arises, Dr. Tenma remains firm and chooses to perform surgery on the young boy Johan Liebert instead of the mayor of the city. Unfortunately, this choice leads to serious branches to Dr. Tenma - Losing your social position being one of them. However, with the mysterious death of the director and two other doctors, the position of Dr. Tenma is restored. Without evidence to condemn him, he is released and continues to reach the position of director of the hospital.

Nine years later, when Dr. Tenma saves the life of a criminal, his past returns to haunt him - once again, he gets face to face with the monster where he was operating. He should now embark on a search to make up for the damage spread over what he saved.

At the end of the manga "Monster", the protagonist Tenma finally finds and confronts Johan, the main villain of the story. Tenma tries to convince Johan to give up his plans of destruction, but eventually has to kill him in self-defense. In the end, Tenma is arrested and convicted of her crimes, but finds peace of mind to learn that she finally managed to end Johan's threat.
